Posted Dec 2011: Ethica Health and Retirement Communities operates 50 nonprofit nursing homes around the state, providing care for almost 6,000 patients. The organization has recently begun examining the problem of re-hospitalizations of Ethica patients. Research has found that rehospitalizations in the nursing home industry are frequent, costly, and often preventable. Almost 25% of patients admitted from a hospital to a skilled nursing facility (SNF) are readmitted to the hospital within 30 days. This cost Medicare $4.34 billion in 2006 alone and in addition is difficult for the patients, who may suffer adverse events such as increased dementia, hospital-acquired infection or fall as a result of unnecessary hospitalizations. Ethica is in the process of implementing an innovative program called INTERACT to address the issues. INTERACT (Interventions to Reduce Acute Care Transfers) works to reduce hospitalizations and re-hospitalizations among nursing home residents through clinical and educational tools and strategies for nursing home staff and physicians. Ethica is currently rolling out the INTERACT program to 12 centers. Students would have the opportunity to assist with project implementation, gather and analyze center data related to hospitalizations and the INTERACT program, and develop resources for other Ethica centers in Georgia who are interested in implementing INTERACT to reduce hospitalizations. Positions are in the fields of microbiology, public health, and health communications. Internship sites include Atlanta, Cincinnati, and San Juan (students must be bilingual in English and Spanish to work San Juan internships). Internship benefits include $470-$570 per week stipend, plus airfare to and from the internship site, and arranged housing. Posted Oct 2011 Community Health Works Cancer Education and Outreach a) Capstone Project 1 The Cancer Coalition Capstone student will operate the Cancer Health Line, conduct intake interviews to provide information for the Health Line database, and provide one on one education about cancer screenings, behavioral changes for the prevention of cancer, referrals to appropriate providers for screening certificates and if necessary the establishment of a medical home. They will also contact existing patients in the database to provide follow-up for those requesting a screening certificate who have not had a physician follow-up request, anyone who has received a certificate but we have not received an invoice for payment or results of screenings and/or additional diagnostic procedures for abnormal findings. Ongoing correspondence and follow-up via phone and email for participants and/or invitees of the Cancer Coalition. The Cancer Coalition Capstone Student will represent CHW at applicable health outreach activities in the community including, but not limited to, health fairs and community resource fairs.
b) Capstone Project 2 Conduct research to update the cancer and health related statistical database to support grant research and writing and community needs assessment. Assist with or conduct community focus groups to support strategic planning processes, community asset mapping and gaps analysis. Develop educational PowerPoint presentations for various audiences with the opportunity to conduct community presentations. Ongoing correspondence and follow-up via phone and email for participants and/or invitees of the Cancer Coalition. Represent CHW at applicable health outreach activities in the community including, but not limited to, health fairs and community resource fairs.
I. Expected Skills and Abilities for All Opportunities: Maintains professional and courteous attitude with all persons within and outside of Community Health Works. Detail-oriented and ability to complete repetitive data entry tasks with precision. Strong public speaking skills (comfortable speaking professionally with individuals of various professional capacities and affiliations). Strong writing skills and confidence to prepare a variety of print communications. (email correspondence, press releases and some web content). Self-starter and ability to multitask and prioritize multiple projects and deadlines simultaneously. Possess a willingness to learn, spirit of flexibility and ability to respond to new tasks and challenges with alacrity. Ability to work independently and as a team member.
II. Qualifications: Education: Pursuing a Master of Public Health Degree (or evidence of candidacy for such) is preferred. Experience: evidence of participation in health outreach, event-planning and/or meeting-facilitation, public speaking, data entry, and written communications experience is desirable. Note: a writing sample may be requested.
III. Technology/Database Proficiency: Proficient with Microsoft Outlook and Microsoft Office including Word, Excel, and PowerPoint
IV. Environmental and special demands: Work in a well-lighted, temperature controlled environment. Must be able to work irregular schedules to attend meetings and promote Community Health Works’ programs and initiatives to the community as needed (health fairs, for example, are often held on weekends). Professional business office attire is required.
